About the work
Chronic-Pharma es una plataforma de ayuda la toma de decisiones (en formato web y app) que integra tres herramientas desarrolladas en el grupo de investigación “Farmacoterapia y cronicidad” de Sevilla. El objetivo final es la mejora de la seguridad del paciente y los correspondientes resultados en salud positivo que les genera, desde el punto de vista de la desprescripción, la identificación de pacientes en riesgo de desarrollar efectos adversos anticolinérgicos y la identificación de señales alertantes que permitan una actuación rápida y dirigida. Las tres herramientas que integra son las siguientes: Anticholinergic Burden Calculator “ABC-Calculator”, los criterios LESS-CHRON y la herramienta TRIGGER-CHRON. La plataforma Chronic-Pharma, se presenta tanto en español como inglés, lo que se traduce en una mayor visibilidad y uso a nivel internacional.
